A major mistake was made on my taxes. Stating I have approximately $45,000 in 401k. I really need to fix this.

    Generally, no.

Generally, for a credit or refund, you must file Form 1040-X within 3 years (including extensions) after the date you filed your original return or within 2 years after the date you paid the tax, whichever is later.

This means that as of March 2025, you can amend your 2021, 2022, or 2023 returns.

Who made the mistake? Your or a tax preparer?

NOTE: there are certain exceptions to the 3-year limit because of federally-declared disasters - please see this IRS page.
