I have an LLC, do I enter my supplemental insurance cost along with the my medicare B & D on the same page?

If your LLC is paying for the insurance then it should be entered in the LLC's tax return as an expense.  If you are paying for the insurance out of your pocket then yes, it will go in medical expenses on your personal return along with your medicare premiums.
